Latrobe Regional Hospital (LRH) is the major provider of specialist health services in the Gippsland region, east of Melbourne. LRH is the regional provider of mental health services in Gippsland. LRH Mental Health Service operates an integrated specialist mental health program across the region and provides inpatient and community mental health care for adults, children and young people and older persons. Gippsland provides a fantastic regional lifestyle with easy access to Melbourne. Bairnsdale is close to Mt Hotham, and Dinner play for those who live the snow. It is also very close to the Gippsland Lakes, and 90 mile beach.
An exciting opportunity has arisen for an experienced and suitably qualified Intensive Complex Care Coordinator to join the Bairnsdale Mental Health Team at Latrobe Regional Hospital on a full time basis.
The successful applicant will be required to: Play an important role in assertive outreach to people with complex issues, who are at risk of readmission to hospital or who have been recently discharged. Work collaboratively with other team members including Community Clinicians
To be successful in this position you will need to have: Well developed organisational skills. If a nurse, Registration with the Nursing and Midwifery Board. Sound knowledge of the Mental Health Act 2014. Ability to assess, and plan care for mental health consumers. Computer Skills.
